There are a various treatment plans for individuals who suffer heartburn or GERD.  Two specific treatment options include the use of over-the-counter medications.  These types of medications include products that are manufactured using calcium carbonate, magnesium and aluminum.  Examples of these products include Tums, Rolaids, etc.Those utilizing prescription medications is another treatment program for heartburn or GERD. The production of acid in the stomach can be stopped by those medications. One of those types of medication are called proton pump inhibitors. The protonix medication is an example of this drug.  Another type of medication may work to minimize the production of acid in the stomach. Cimetidine is an example of this type of medication.

The U.S. Congress Sends Mixed Message On Indonesian Military
The U.S. Senate voted against giving the human rights abuses countering the Houses vote to reinstate funding and training for the Indonesian military.
President George W. Bush and Condoleezza Rice had been pushing for a full restoration of all military ties to Indonesia because of the ongoing terrorist threat but the persistence of human rights abuses and the corrupt nature of the Indonesian military and political system are problematic when considering the legitimacy of such an idea.
The Indonesian military has a long way to go before it becomes an accountable institution that respects human rights and civilian authority, states Karen Orenstein of East Timor and Indonesia Action Network (ETAN)
The U.S. House of Representatives had passed a bill in June restoring full military aid to Indonesia. On this, Republican congressman from West Michigan, Pete Hoekstra says, U.S. policy towards Indonesia focuses on assisting and providing humanitarian assistance. Lingering concerns exist regarding human rights issues and violence in places such as Aceh and West Papua. In 2005 the Secretary of State (Condoleezza Rice) certified that Indonesia has met the requirements for International Military Education and Training [IMET).
IMET parallels the School of The Americans now The Western Hemisphere Institute for Security Cooperation both training schools having reputations for churning out soldiers and police that frequently turn up on lists of human rights abusers or worse. While IMET training takes place in the U.S. another program, Joint Combined Exchange Training [JCET] sends special forces teams to other countries, particularly Indonesia. Chalmers Johnson, in his book cites JCET training as a direct link to Indonesian rapes and murders of civilians in Indonesia. Johnsons more recent book, The Sorrow of Empire, gives an updated account of U.S. empire.
The U.S.s historical relationship with Indonesia is a sordid one. It first came to light when Alan L. Pope, an American pilot, was shot down while on a bombing run over the Indonesian archipelago, after had bombed a church and an outdoor market on May 18, 1958. The U.S. had orchestrated B-26 bombing and strafing raids on the civilian population in an attempt to destabilize the Sukarno government. The number of civilians deaths appear to have been significant but actual numbers have never been established.
In 1962 the Kennedy administration informed Sukarno that the U.S. would support the Indonesian militarys invasion and occupation of West Papua. Seven years later the Nixon administration recognized the results of a rigged, fraudulent vote in which 1,022 people voted to join Indonesia, in a population of over 800,000. The U.S. recognized this as a legitimate agreement of the Papuan population to join Indonesia. In government documents declassified last year and available on the National Security Archive web site it is shown that the Indonesian takeover of West Papua and fraudulent vote, dubbed a free choice by Indonesia and the U.S. was illegitimate and a deception. For 36 years now West Papua has been exploited and abused by the Indonesian military, International Corporations and almost continuous support of the U.S. government. Murders and rapes and forced removal from their land at the hand of the military was becoming commonplace. A U.S. corporation with close ties to the military and notoriety because of its treatment of its workers and the surrounding population in Papua is Freeport, a copper and gold mining company associated with Henry Kissinger.
In 1965 the U.S. played a significant role in the overthrow of Sukarno and the subsequent murders of over 500,000 people. In one of the worst mass murders in world history within a short period of time huge numbers of people, sometimes whole families were rounded up and evacuated. Many were members of the PKI, Indonesias Communist party with an estimated 1 million members, others were random victims. Hundreds of thousands became political prisoners. One of those, Pramoedya Ananta Toer, who was sympathetic with the PKI, was placed in an island concentration camp for 17 years. Pramoedya is Indonesians most famous writer. To this day, those involved in this crime against humanity remain free.
The military and Suharto had said their justification for their massive crime was an iminent takeover of Indonesia b y the PKI. In recent years documents have proven this excuse to have been a fabrication. From 1965 until 1999 the Indonesian dictatorship had received the support of the U.S. government.
Ten years after the mass murder of over half a million leftists, East Timor, having just received her independence from being a Portuguese colony was invaded by the Indonesian military, and again, with the blessing of the U.S. government. Gerald R. Ford and Henry Kissinger gave the green light and subsequently shipped the Indonesians M-16 rifles and other military equipment. Result, forcible takeover of a small country of 800,000 people, mostly very poor. Within a short period of time 200,000 civilians were killed in the takeover largely with the U.S. supplied rifles. The U.S. supported the events in East Timor until 1991. When an infamous massacre occurred that was covered by the world press only then did the U.S. government restrict some weapons from going to the Indonesian military. In 1998 an Australian newspaper, The Age, had a small report of the discovery of oil in East Timor territorial waters. In 1999 when East Timor voted for independence from Indonesia the U.S. and Australia, Britain and the U.N. supported its move for independence. The Indonesian response was to burn down and destroy as much of the newly independent country on their way out.
Aceh in Indonesias north west is a mostly poor, primarily Muslim area that was hardest hit by last years Tsunami. Fishing was the primary source of income for many Acehanese when the Tsunami hit. Over 100,000 were said to be lost in the area.
About 30 years ago oil and natural gas deposits were discovered in the Aceh region but the people never saw any of the benefits. an Aceh independence movement was formed, GAM. That has been battling the Indonesian government since 1976 with over 10,000 deaths related to the conflict. A peace agreement was just signed but it is unknown if it will hold. No doubt the U.S. and Australia have a keen interest in access to the large oil and natural gas deposits in the area.
The Bush administration and those in congress who wish to fully normalize relations with the Indonesian government and restore weapons sales and training to the Indonesian military would be to help in propping up and giving assistance to a corrupted system. This would not be anything new. The U.S. has been doing this with Indonesia for 50 years. The approximately one million deaths in Indonesia committed by the Indonesian military from 1965 on, in a large part had to do with U.S. hegemony, hubris, and greed. OK. Palliation means making the patients illness get better while paying attention to the surrounding psycho-social and spiritual issues. Its what we should be doing for all our patients all the time, so why does a hospital need a special service? (The focus on patients with advanced illness doesnt really narrow things down much; most hospitalized patients have advanced illness). My reading suggests that palliative care is simply hospital medicine raised to a higher level of excellence, efficiency, organization and patient centeredness. It sounds a lot like what the Institute of Medicine has been harping at all of us to do. The idea of a palliative care suggests a separate team to deliver this excellent care while the rest of us settle for mediocrity! Palliative care claims to have distinctive attributes, but they really shouldnt be distinctive. They should be the norm.

A common thread in many descriptions of palliative care relates to the quality of communication. Discussions go beyond DNR status and whether the patient has a living will to the education of patients and families on the big picture of the patients health and realistic expectations concerning the limitations of treatment. This leads to greater patient satisfaction, more informed and rational decision making, and often the avoidance of ICU care, feeding tubes and other technologies which, in many patients, serve only to decrease quality of life. While these interventions are ineffective in many patients they are costly, which is probably why palliative care programs save hospitals money. Lots of money. But again, why should good communication with patients and families require a special care service?

Like so many other concepts in medicine, the word has been hijacked. The idea of palliative care has been administratively defined by people with agendas. For clinicians its been an exercise in obfuscation.

The excellence and the efficiency of what is called palliative care is something to get excited about. Its something we should all strive for. I would like to see our hospitalist program spearhead a multidisciplinary effort to take these ideas to a new level of excellence for all of patient care. But I dont think Id call it palliative care. Id call it something else. Im not sure what, but something with more meaning.

It sounds better without them.

It's like America went through menopause and could only listen to this kind of sappy pap. I hate this thing, and thanks to the Crap and Crap Lite stations being played where I worked constantly, I heard it way too many fucking times.
My Favorite Nominee: MMMBop, Hanson. It's a default choice; it's the one I think is okay whereas I despise all the others (especially "Where Have All the Cowboys Gone" by Paul Cole, which should be classified a form of abuse).

Winner: Change the World, Eric Clapton. I'm not much of a Clapton fan, really. I did like this song, although it's association with the awful Scientology-promoting John Travolta film Phenomenon counts against it a tad. It's not earth-shaking, but it's a solid, not-unpleasant song.

My Favorite Nominee: Losing My Religion, R.E.M. It was overplayed, but if you listen to it now, it sounds almost fresh again. It really is just a good song.

Winner: Another Day in Paradise, Phil Collins. Preachy, annoying, and not even the best song from that Phil Collins album. (Actually, I just checked and sadly, it is. I despise "Something Happened on the Way to Heaven," and "I Wish It Would Rain" just sounds like a rip-off of "Wish You Were Here" with Clapton on guitar.)
My Favorite Nominee: Nothing Compares 2 U, Sinead O'Connor. Of the sappy, preachy, sad sack songs that were nominated this year, this is the one that's actually a good song. (Also, "U Can't Touch This" was nominated this year, but come on, man.)

Winner: Wind Beneath My Wings, Bette Midler. I hate this song, and my dad pissed me off by playing it at his wedding reception for his mother, which I specifically told him not to do because it was such a fucking cliche. He said he wouldn't; he did. Wow, my grandma must have been one of 10 million special woman so uniquely honored that year. It's the equivalent of buying your dad a tie on Father's Day.
My Favorite Nominee: The End of the Innocence, Don Henley. Chance is right on when he calls it deceptively angry. It adds some world-weariness on top of that, too. Beautiful song.
My Favorite Single That Year: A Little Respect, Erasure. I usually come out on the side of pop, I guess. Although besides the catchiness, I think the lyrics are beautiful. One of my all time favorite lyrics comes from this song: "What religion or reason could drive a man to forsake his lover?"

"Strong Man Acts" moved to North America in the 1800's and are credited with inspiring what is known today as bodybuilding. In that day, a strongman meant someone who displayed his physical prowess, ordinarily as a circus stunt man. If you wanted to see a strongman performance, including bench pressing, steel bending and breaking iron chains, you would have to go to the circus. In the 19th century you could purchase pictures of many of the well known strongmen of the time, bodybuilding did not exist yet, however it did not stop them from flexing and posing for pictures. Today, the word "strongman" is associated with contestants in various contest of strength. (rock lifting, pulling or towing large trucks etc).

Using a combination of different exercises to develop physique and muscle fiber is what is referred to as bodybuilding. (mainly weight training) as well as diet plan. However, not every bodybuilder is in it for the competition. Points are assigned based on muscle tone, muscle mass and muscle definition as well as appearance by the judges of the spot.

* Sandow's counterpart in North America was a man named Bernarr Macfadden. Macfadden put forth a great deal of effort to increase his strength (he was not healthy during childhood), and began by offering fitness apparatus for sale. He also advocated women's physical fitness (this was considered a great new idea at the time). The first publication of his "Women's Physical Development" magazine arrived in 1900. It was renamed Beauty and Health shortly thereafter. Macfadden set up several healthatoriums (institutes that offered courses in physical fitness) throughout the eastern as well as midwestern parts of America.

* The first bodybuilding competition in America happened on January 16, 1904 and was sponsored by Macfadden.S at Madison Square Garden, New York City. Two of the terms used to describe it were Physique Contest and Physique competition (other than weight competitions, strength exhibitions or athletic feats) was the focus of the show. The winner, Al Treloar, was declared The Most Perfectly Developed Man in the World. Charles Adams is one of the well known competitors who achieved bodybuilding fame, he did so in the years of 1921 and 1922.

* The initial significant worldwide bodybuilding contest was the Mr. Universe competition, established in 1947, then the Mr. Olympian contest that was in 1965.

The period between 1940 and 1970 is known as the Golden Age of bodybuilding because this is when the criterion of more muscle mass was added to the ideal bodybuilder aesthetic, along with the existing criteria of muscular symmetry and definition. World War II is often sited as the chief reason towards the growth of larger, powerful, boldly assertive attitudes. The 1977 film, Pumping Iron, starring Arnold Schwarzenegger also gave bodybuilding added publicity.

* Women's bodybuilding competition began in the 1970s (although a few women had participated in earlier contests organized by Macfadden in the U.S.)

* The International Olympic Committee granted provisional status to the sport of competitive bodybuilding in 1998 but has not yet approved it.

* With advancements in technology, bodybuilders are more massive today than they have ever been. Many competitors in today's bodybuilding competitions weigh over 250 pounds and have less than 5% body fat.
